Publish a form
Dex Forms collect requests and records. Every submission waits for review before it becomes a record.
1Build the form
Add up to 64 fields from 13 types, including text, numbers, dates, choices, email, phone, links, and file uploads. Mark required fields and set the accepted file types for each upload, up to 50 MB per file.
2Publish or invite
Choose Publish form for a public link, or invite one person by email with an expiry date and a limit on submissions. An invited person signs in with the address you invited.
3Embed it
Turn on Enable embed and list the sites allowed to show the form, up to 32.
4Review submissions
Open each submission and choose Accept submission or Reject submission, with a review note. Accepted submissions become records.
5Take it down
Choose Revoke publication to stop accepting submissions.
